Is the cable car / entrance ticket included?

No — Ba Na entry includes cable car and is around $40 per adult, paid at the gate. The private-driver day rate covers the car and driver only.

How is this different from a Ba Na group tour?

Group tours give you ~4 hours up top and force you onto a fixed cable-car return. Private gives you 6+ hours and lets you leave when you decide.
